Excavation/Penetration Hot works Permit to Work Electrical Work Mechanical Isolation Confined Space High Voltage Working & Heights Complex Lift Permit Permit to Clear/Disturb Add into the new permit Authorized Permit Issuer: Name/Date /Time Permit assigned to an authoriized Permit Holder - Name/ Date /Time Associated JSA's dropdown from the JSA Register, All permits require a JSA. Tasks & Permit Completed by Holder: Name/Date /Time/Signature. Permits also need to be Lifted/Closed with a Permit Issuer - Date/Time and by the Name / Signature.
